In July of 2005, “the supervisor” conducted an audit of KTR’s NDT and welding qualification system. Subsequent to the assessment, KTR’s new level III examiner continued further review and found that a level II NDT inspector did not have a current certification. 443 piping joints were immediately de-certified and a Method “D” Corrective Action Request was issued.
A Method “D” CAR comes from the C.O. of the base. It essentially means, fix it, and/or get the fuck off my base. There isn’t an “E”.
KTR’s Q.A. program was also de-certified and the contractor was unable to perform ANY work for the supervisor for 4 months.
Several Major Non-conformities were issued to keep the fleet running. KTR’s first action was to de-certify all NDT inspectors and re-train everyone.
If that isn’t a million dollar fuck-up, I don’t know what is. A “near miss” can cost 30 people, 3 hours each. I can only imagine the amount of blood on the floor after something like that.
Ahhhh, the joy of government contracting.
